Hello, I’m Dr. Cha-Young Kang, chief director of NOSELAB Clinic.
Today, I’d like to share a revision rhinoplasty case focused on high nasal bridge correction and crooked nose improvement using autologous rib cartilage.
Patient Concerns Before High Nasal Bridge Correction
The patient underwent rhinoplasty in 2013, which likely involved a silicone implant, septal cartilage graft, and hump reduction. Over time, she noticed her nasal tip lifting, raising concerns of contracture.

Her concerns included:
Nasal bridge starting too high between the eyes
Crooked nasal line and asymmetry
Upturned tip with nostril exposure
Straight, rigid appearance she described as “chalk-like”
Triangular nasal wings and tip
Desired a softer, rounded tip with feminine balance
Wanted nasolabial angle adjustment to shorten the philtrum
Her goals were to correct the crookedness and lower the high nasal bridge starting point for a naturally feminine look.

Surgical Plan for High Nasal Bridge and Crooked Nose Correction
Septal Extension with Rib Cartilage – Corrected the upturned tip, rebuilt stable support, prevented contracture.
Lowering of the Nasal Bridge Starting Point – Reduced the glabella height, sculpted a natural curve.
Correction of Crooked Nose – Lateral osteotomies straightened the dorsal line and improved symmetry.
Nasolabial Angle Adjustment – Brought the tip down, reduced nostril exposure, shortened philtrum visually.
Tip Refinement – Smoothed triangular tip into a rounded, feminine shape.
Functional Improvement – Inferior turbinate reduction improved rhinitis symptoms and breathing.
Results After High Nasal Bridge Correction and Feminine Refinement

Frontal View: Crooked nose corrected, nostril exposure reduced, feminine nasal line achieved.

Side Profile: High bridge starting point lowered, tip repositioned, philtrum visually shortened.

Overall: Transitioned from masculine impression to soft, feminine look with balanced contours.

Medical Insight
This revision rhinoplasty required caution due to potential contracture and scar tissue. Using rib cartilage provided stable support while lowering the high nasal bridge starting point—a technically demanding step.
Adjusting the nasolabial angle and refining the tip delivered the rounded feminine result the patient desired.
At NOSELAB Clinic, we provide customized surgical plans balancing aesthetics with function to achieve long-term outcomes.
Thank you for reading.
