Introduction to the L-Shaped Silicone Implant Revision Case

Hello, this is Dr. Cha-Young Kang from Nose Lab Clinic.

Today, I will discuss a revision rhinoplasty case where a patient underwent L-shaped silicone implant revision, correcting functional issues and restoring a natural nasal structure.

Patient History and Concerns

📍 Surgical History

  • 13 years ago: Filler injection

  • 13 years ago: First rhinoplasty (silicone + ear cartilage)

  • 6 years ago: Revision with L-shaped silicone implant

  • 5 years ago: Additional revision due to persistent swelling

🔎 Initial Concerns

1. Functional Problems

  • Nasal congestion and alternating obstruction

  • Difficulty breathing

  • Chronic runny nose

2. Structural Issues

  • Over-rotated, upturned tip

  • Triangular and asymmetric nostrils

  • High radix creating unnatural appearance

Surgical Plan – Focused L-Shaped Silicone Implant Revision

💉 Key Procedures

1. Nostril Correction

  • Cartilage grafting to reinforce internal support

  • Skin grafting to improve nostril shape

  • Tip rotation correction to reduce nostril show

2. Upturned Nose Correction

  • Lowering the radix

  • Balancing nasal projection for facial harmony

3. Functional Restoration

  • Septoplasty for deviated septum

  • Treatment of hypertrophic rhinitis

  • Improved airflow and nasal comfort

Surgical Outcome After L-Shaped Silicone Implant Revision

Results Overview

Frontal view comparison before surgery (left) and immediately after revision rhinoplasty (right), showing improved nostril shape and nasal symmetry.
Before Surgery (Left) / Immediately After Surgery (Right)

Frontal View:

  • Corrected triangular nostrils

  • Improved symmetry and reduced visibility

  • Natural contour restored

Side profile comparison before surgery (left) and immediately after revision rhinoplasty (right), showing lowered radix and corrected upturned nasal tip.
Before Surgery (Left) / Immediately After Surgery (Right)

Side View:

  • Lower radix with balanced nasal line

  • Improved nasolabial angle and profile

Oblique side view comparison before surgery (left) and immediately after revision rhinoplasty (right), showing improved nasal projection and reduced nostril exposure.
Before Surgery (Left) / Immediately After Surgery (Right)

Functional:

  • Reduced nasal discharge

  • Easier breathing

  • Stable septum structure

In this case, the patient had multiple surgeries over 13 years.

The most critical issue was the inappropriate L-shaped silicone implant, which caused over-rotation, nostril exposure, and functional discomfort.

We reconstructed the nasal structure using autologous rib cartilage and I-shaped grafts, restoring both aesthetics and function.

Functional correction was achieved through septoplasty and treatment for chronic rhinitis, significantly improving breathing.

At Nose Lab Clinic, all rhinoplasty procedures are performed using the closed technique only.
