Hello, this is Dr. Chayoung Kang, Director of Noselab Clinic, specializing in closed rhinoplasty and revision nose surgery in Korea.
Today, I will share a real case of long and deviated nose correction with visible cartilage improvement and nostril asymmetry correction.
Preoperative Consultation for Long and Deviated Nose Correction
The patient visited our clinic after receiving a previous surgery at another clinic.
Her nose appeared unnaturally long and deviated, and the cartilage of the nose tip was visible through the skin, making the overall look artificial.
In addition, the nostril asymmetry became more pronounced because of the distortion caused by the visible cartilage.
Surgical Plan to Correct Visible Cartilage and Septal Deviation
A CT scan revealed that the patient had a septal deviation, and the misalignment of the nasal septum was directly related to the nostril asymmetry.
Therefore, I planned to perform a septal deviation correction while adjusting the nasal framework.
By correcting the deviated septum and reshaping the nasal tip, the goal was to improve both the functional balance and aesthetic proportion of the nose.
Revision Rhinoplasty Findings and Results
During the surgery, it was confirmed that the previous septal extension graft had displaced the front part of the septum.
This caused the nose tip distortion, columella tilt, and nostril asymmetry.
The first half of the septal cartilage was repositioned to its proper location, and the visible cartilage at the nasal tip was corrected by covering it with soft tissue and precisely repositioning the cartilage.
After the correction, the nostrils appeared more symmetrical, and the nasal tip line looked smoother and more natural.

General Overview – Improving Nostril Asymmetry and Tip Balance
In revision rhinoplasty cases, nostril asymmetry often stems from septal deviation or cartilage displacement.
Especially when a septal extension graft has been used in previous surgeries, its misalignment frequently leads to visible cartilage, tilted columella, and asymmetric nostrils.
By reconstructing the deviated septum and realigning the nasal structure, both the function and aesthetics were restored in this case,
achieving a more natural and balanced nasal appearance.
