Hello, I’m Dr. Cha-Young Kang, Director of Nose Lab Clinic.

Today, I’ll share the story of a 31-year-old male patient who underwent multiple nasal surgeries that left him with a longer, asymmetrical nose. This Long Nose Correction case highlights the importance of a tailored approach in rhinoplasty revision to achieve both functional and aesthetic harmony.

1. Patient History and Revision Background

  • 2015: Initial surgery with silicone implant, nasal bone reduction, donated rib cartilage graft

  • 2019: Nostril reduction

  • 2023 (May): Silicone replacement, nasal bone reduction, additional rib cartilage graft

  • 2023 (October): Additional revision due to nasal deviation

  • 2024 (March): Implant-free surgery (SR technique) and nostril lift, with foreign material removal

Additional Treatments:

  • 18 stem cell therapy sessions at other clinics before and after surgeries

Main Concerns:

  • Elongated nasal appearance (long nose)

  • Crooked nose and nasal asymmetry

  • High nasal starting point at the glabella

  • Nostril asymmetry

2. Pre-Surgical Analysis for Long Nose Correction

Preoperative design consultation photos showing the patient’s right side, oblique side, and nostril view before long nose correction rhinoplasty.
Design Consultation: Right Side View, Oblique Side View, and Nostril View Photos
  • Front View: Broad bridge, nostril asymmetry, tip deviation

  • Side View: High starting point, elongated nasal length, sharp nasal tip

  • Nostril View: Tilted columella, nostril asymmetry, visible scarring

3. Surgical Plan and Procedures

1. Structural Reconstruction:

  • Septal Extension with Autologous Rib Cartilage to support tip

  • Septal Deviation Correction for straightening and airflow

  • Right Nasal Wing Reconstruction for symmetry

2. Asymmetry Correction:

  • Nostril Lowering on the right side with graft

  • Right Alar Lift Surgery

3. Foreign Material Removal:

  • Removed irregular rib cartilage particles

4. Adjustment of Nasal Starting Point:

  • Bone contouring at glabella

  • Soft tissue thinning to lower starting point

5. Finishing Touches:

  • Custom-carved thin silicone implant for natural contour

4. Surgical Outcomes of Long Nose Correction

  • Front View: Improved nostril and wing symmetry

  • Side View: Reduced nasal length, lowered starting point, natural profile

  • Nostril View: Balanced symmetry and columella alignment

Key Achievement – Lowering the Nasal Starting PointThe patient’s overly high glabella starting point was corrected with careful bone refinement and soft tissue adjustment, a crucial step in this Long Nose Correction case.

Conclusion

This case demonstrates the challenges of Long Nose Correction in revision rhinoplasty, where multiple prior surgeries often complicate both aesthetics and function. At Nose Lab Clinic, we specialize in complex cases requiring structural reconstruction and asymmetry correction to restore balance.
