Pre-surgery Evaluation and Design for Long Nose and Retracted Columella
The patient presented with a long nasal tip and retracted columella, resulting in an overly sharp nasal angle and a downward impression when viewed from the side. This condition can make the face appear longer and the nose overly dominant. The main goal of this Long Nose and Retracted Columella Correction was to reduce the nasal length, restore the ideal nasolabial angle, and create a softer facial balance.
Surgical Plan for Long Nose Correction and Columella Advancement
The procedure was performed using a Closed Rhinoplasty approach to avoid external scars.During surgery, the downward-rotated nasal tip was adjusted upward to shorten the overall nose length. At the same time, retracted columella correction was achieved by advancing the columellar strut and extending the nasal septal support using autologous rib cartilage.
This Long Nose Correction not only improved nasal proportion but also refined the angle between the columella and upper lip, resulting in a more natural, gentle appearance.
Post-surgery Results and Specialist Review

Photos were taken immediately after surgery with the patient’s consent.
Frontal view: Balanced nasal proportions and natural tip alignment
45° and side views: Shorter nasal length and improved columella projection
Basal view: Enhanced symmetry and corrected retraction
After the operation, the long nose and retracted columella were successfully corrected, producing a softer and more proportionate nasal line. This case demonstrates the advantages of Closed Rhinoplasty Korea for precise correction and minimal recovery time.
