Hello, this is Dr. Cha-Young Kang, Chief Director of NoseLab Plastic Surgery in Seoul.


Today’s case presents a complex revision rhinoplasty involving Medpor removal and complete septal reconstruction using autologous rib cartilage.The patient came to our clinic suffering from nasal obstruction and aesthetic imbalance caused by a previously inserted Medpor implant.


Understanding the Patient’s Condition and the Impact of Medpor

  • 10 years ago: Primary rhinoplasty using silicone and cartilage

  • Medpor was used for septal extension

  • Current symptoms: Nasal obstruction (left > right) and chronic rhinitis


While Medpor implants may appear convenient initially, they can cause long-term

complications such as inflammation, infection, or extrusion.Because of these risks, I do not recommend Medpor for structural nasal support, particularly in the septum.

Surgical Plan – Safe Medpor Removal and Septal Reconstruction


1️⃣ Complete Removal of Medpor

The Medpor implant had adhered tightly to the surrounding tissue, weakening the septal structure.

It was fully removed under microscopic visualization to minimize mucosal damage and prevent postoperative scarring.

2️⃣ Septal Reconstruction Using Autologous Rib Cartilage

After removing Medpor, the septum was rebuilt entirely with the patient’s rib cartilage.

This provided strong, biologically compatible support and eliminated the risk of foreign-body reaction.


3️⃣ Alar Cartilage Reconstruction

Both alar cartilages were severely damaged and partially absent.

They were reconstructed using rib cartilage to create a natural nasal tip line and restore stability.


Functional Restoration After Medpor Removal


In addition to the structural correction, the patient also experienced nasal obstruction and rhinitis.To restore proper breathing, we performed:

  • Septoplasty for deviated septum correction

  • Submucosal resection of the inferior turbinates using radiofrequency laser

  • Adhesiolysis for intranasal scar release


After Medpor removal, nasal airflow was immediately improved, and rhinitis symptoms subsided significantly.


Aesthetic Refinement and Facial Balance


To enhance overall harmony, the bridge starting point was lowered and softened, and the nasolabial angle was refined.

This adjustment reduced the appearance of a protruding mouth and created a balanced, natural profile.

While Medpor can initially maintain shape, over time it may weaken surrounding tissues or cause inflammatory complications.
