Mesh Removal Rhinoplasty: Achieving a Naturally Subtle Nose with Precision

Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic. Today, I’d like to share a case of mesh removal rhinoplasty, highlighting how we restored a patient’s natural beauty after complications caused by mesh.


 Patient’s Initial Condition Before Mesh Removal Rhinoplasty

A young woman in her twenties came to our clinic, having previously undergone rhinoplasty in 2018. Her initial surgery involved septal cartilage, mesh insertion, and osteotomy. This time, she wanted a refined, “presence-free” nose that looked natural and harmonious.

Surgical Plan for Mesh Removal Rhinoplasty

To address these concerns, we developed a detailed plan:

  1. Lateral osteotomy to narrow the nasal width

  2. Bridge height adjustment for better facial proportion

  3. Tip refinement with alar cartilage repositioning and suture

  4. Nostril asymmetry correction with septal reconstruction and rib cartilage extension

  5. Nasolabial angle correction with subtle dorsal augmentation using rib cartilage


Intraoperative Findings – Mesh Removal Challenges

During surgery, we discovered four pieces of mesh (two on each side) firmly adhered to surrounding tissue. This required delicate dissection to minimize trauma.


Additionally, the septal cartilage had been severely damaged in the previous surgery, making rib cartilage reconstruction essential.

Post-Surgical Results of Mesh Removal Rhinoplasty

Despite the complexity, the results were highly satisfying:

  • Refined nasal tip instead of a blunt appearance

  • Smooth, natural nasal bridge line

  • Lowered nasion for balanced facial proportions

  • Corrected columella for a harmonious profile
