Case Overview – Addressing Nostril Asymmetry and Upturned Tip
Hello, this is Dr. Cha-Young Kang, Director of NOSELAB Clinic.
Today, I’d like to share the case of a patient who underwent multiple surgeries, including alar base reduction, and later returned for a nostril asymmetry correction and upturned nasal tip revision using autologous rib cartilage.
📍Surgical Background

Surgical History
2014: Deviated septum correction + rhinitis surgery + silicone dorsal implant + septal extension (later TP revision)
2015: Nose bridge, tip, and philtrum filler injection (dissolved 3 years later)
2018: Alar base reduction
2021: Philtrum shortening surgery
🔎 Preoperative Analysis

Frontal View
Bulbous nasal tip
Asymmetric nostrils
Deviated nose
Side View
Upturned nasal tip
Prominent upper lip
Irregular nasal dorsum
Hump-like appearance
Oblique View
Upturned, bulbous tip
Protruding upper lip
Nostril View
Clear asymmetry due to prior alar reduction
Visible scar tissue at nostril base
Additional Findings
Preexisting rhinitis
Narrower right nasal cavity
✨ Patient’s Concerns and Goals
Correct drooping alar contour
Smooth out step deformity from implant
Realign deviated nose
Achieve nostril and alar rim symmetry
Improve nasolabial angle
Asked about scar revision (not recommended due to risk of worsened scarring)
✅ Surgical Strategy and Results

Frontal View
Straighter nasal alignment
Balanced nostril shape
Improved nasal refinement

Side View
Lowered nasal tip
Smoother dorsal contour
Reduced upper lip prominence

Nostrils View
Improved nostril symmetry
Alar rim correction

👨⚕️ Surgeon’s Insight on Nostril Asymmetry Correction

This case presented complex challenges due to prior alar base reduction and philtrum shortening. The nostril asymmetry and visible scarring were major concerns.
Rather than revising the scar (which could worsen it), we focused on structural repositioning to visually improve nostril asymmetry. Rib cartilage was used for nasal tip support and dorsal reshaping. Functional concerns such as narrow nasal passage and rhinitis were also addressed.
In the end, the patient achieved better nostril balance, a refined tip, and a more harmonious nasal contour.
