Introduction to Post-Orthognathic Septal Reconstruction

Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.

Today, I’d like to introduce a Post-Orthognathic Septal Reconstruction and refinement case for a patient who underwent orthognathic (double jaw) surgery, which led to septal damage, recurrent rhinitis, and nasal asymmetry. 🔹 2019: Septoplasty + Ear Cartilage Graft + Silicone Implant + Functional Rhinitis Surgery

4. Surgical Plan & Approach for Post-Orthognathic Septal Reconstruction

Septal Reconstruction

  • Reconstructed the damaged septum using donated rib cartilage after post-orthognathic nasal trauma.

  • Performed septal extension grafting to enhance nasal tip projection & support.

  • Lateral osteotomy to align the nasal bones and improve symmetry.


Functional Enhancement

  • Corrected the deviated septum to restore balanced nasal airflow.

  • Performed radiofrequency turbinate reduction (submucosal resection) to treat hypertrophic rhinitis.

  • Ensured long-term stabilization of nasal function.


Aesthetic Refinement

  • Performed alarplasty on the right side to correct congenital nostril asymmetry.

  • Reduced the nasal base width while maintaining natural proportions.

  • Created a sleek, structured "straight bridge line" that harmonizes with the patient's facial features.
