Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.

Today, I’ll share the case of a patient who came to us with concerns about nostril asymmetry and nasal obstruction, requiring a carefully planned revision rhinoplasty for nostril asymmetry. In this patient’s case, previous surgeries had caused internal nasal adhesions, which led to severe nasal blockage.

1. Surgical History and Goals in Revision Rhinoplasty for Nostril Asymmetry

Surgical History:

  • 2021: Ear cartilage and filler injections

  • 2022: Septal cartilage, autologous rib cartilage, and Gore-Tex implant

  • October 2022: Philtrum reduction

  • 2023: Augmentation surgery

Patient’s Goals:

  • Address nasal obstruction on both sides

  • Correct nostril asymmetry

  • Reduce nostril visibility

  • Achieve a natural, refined nasal appearance

2. Pre-Surgical Analysis in Revision Rhinoplasty for Nostril Asymmetry

Pre-surgery consultation photos of a female patient: frontal view, left side view, and left oblique side view before revision rhinoplasty.
Design consultation photos: Frontal view, left side view, and oblique side view.
  • Front View: Tip deviated to the left with noticeable asymmetry in nostril shape and visibility.

  • Side View: Drooping tip, dorsal hump, high starting bridge point, and wide nasolabial angle.

Pre-surgery consultation photos of a female patient: right oblique side view, right side view, and nostril view before revision rhinoplasty.
Design consultation photos: Right oblique side view, right side view, and nostril view.
  • Nostril View: Columella deviated slightly right, left nostril smaller and longer.

  • CT & endoscopy confirmed deviated septum and mucosal adhesions.

3. urgical Plan – Revision Rhinoplasty for Nostril Asymmetry and Function

  • Septal Extension with Autologous Rib Cartilage: Stabilize and refine nasal tip.

  • Septal Reconstruction: Shift septum to the right for improved airflow.

  • Nostril Lengthening: Reposition and reinforce alar cartilage to correct asymmetry.

  • Replacement of Gore-Tex with Silicone: Natural alignment with lower starting bridge.

  • Alar Cartilage Suturing: Refine tip shape.

  • Nasolabial Angle Adjustment: Correct angle for balance.

4. Post-Surgical Results

Side view comparison before (left) and immediately after surgery (right), showing correction of drooping tip and balanced nasal profile.
Before Surgery (Left) / Immediately After Surgery (Right)

Side View:

  • Drooping tip corrected, dorsal hump removed, balanced angle.

Oblique side view before (left) and immediately after surgery (right), highlighting refined nasal bridge and natural contour.
Before Surgery (Left) / Immediately After Surgery (Right)

Oblique View:

  • Bridge aligned with pupil line, natural harmony achieved.

Frontal view comparison before (left) and immediately after surgery (right), showing improved nasal tip alignment and nostril balance.
Before Surgery (Left) / Immediately After Surgery (Right)

Front View:

  • Nostril visibility reduced by more than half, asymmetry corrected.

Nostril view comparison before (left) and immediately after surgery (right), showing corrected nostril asymmetry and columella alignment.
Before Surgery (Left) / Immediately After Surgery (Right)

Nostril View:

  • Septal reconstruction corrected nostril asymmetry, columella aligned.

5. Conclusion

This case shows that revision rhinoplasty for nostril asymmetry requires both functional and aesthetic correction. Choosing an experienced team is crucial, especially for cases involving nasal obstruction and septal reconstruction.

At Nose Lab Clinic, we are committed to achieving safe, natural, and balanced results for patients with nostril asymmetry.

All surgeries at Nose Lab Clinic are performed using a closed approach.

Postoperative precautions and possible complications after rhinoplasty – includes information on infection, bleeding, asymmetry, allergic reactions, and scarring.
