Today, I’d like to share a particularly meaningful Revision Rhinoplasty case — a patient who had undergone multiple nose surgeries over nearly two decades. Over the past 19 years, the patient had a complex surgical history:

  • 19 years ago: First rhinoplasty for droopy nose correction

  • 10 years later: Revision with Gore-Tex implant → inflammation after 5 years

  • 3 years ago: Implant removal

  • 2 years ago: Revision with silicone + donated dermis

  • 1 year ago: Columella reconstruction

  • 5 months ago: Additional revision at another clinic


She also presented with septal perforation and scar tissue from previous surgeries.Her main goals were to achieve a natural nasal contour, correct the collapsed columella, and maintain long-term stability without inflammation.

Surgical Plan – Rib Cartilage Reconstruction for Structural Stability


Complete Revision Rhinoplasty was performed using autologous rib cartilage to restore both form and function.


1. Complete Septal Reconstruction Rebuilt the entire nasal septum using rib cartilage to strengthen internal support and correct deviation.


2. Removal of Damaged Tissue and Mucosal Repair Removed exposed materials and repaired damaged nasal mucosa.


3. Septal Perforation Repair Closed a 7 mm perforation with auricular perichondrium to restore healthy tissue continuity.


4. Alar Cartilage and Columella Reconstruction Both alar cartilages were replaced with rib cartilage to create a well-defined, natural nasal tip.


5. Bridge Augmentation Avoided synthetic implants; rebuilt dorsal height solely with rib cartilage for durability and safety.
