Upturned Nose Correction for a More Elegant and Stable Nasal Shape

Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.

Today, I’m sharing the case of a patient who underwent multiple rhinoplasty procedures, resulting in structural deformation. This revision surgery aimed not only to correct existing issues but also to achieve a more elegant and harmonious nasal shape through precise upturned nose correction techniques.

1. Surgical History

Patient’s Background Before Upturned Nose Correction

  • Unknown: Initial surgery

  • 8 years ago: Revision surgery

  • September 2019: Revision causing bulbous tip & drooping tip

  • 2020: Donated rib cartilage + silicone implant

  • 2021: Ear cartilage graft

2. Patient’s Goals

Desired Outcomes of Upturned Nose Correction

  • Reduce excessive nostril visibility in frontal view

  • Elevate the nasal tip

  • Enhance the height of the nasal starting point (bridge near glabella)

  • Address volume deficiency on the left nasal dorsum

  • Achieve natural, subtle volume enhancement for refined results

3. Preoperative Analysis

Structural and Functional Issues Before Upturned Nose Correction

  • Weak nasal tip support from multiple prior surgeries

  • Significant nostril asymmetry

  • Volume deficiency on left nasal dorsum

  • Visible scars and skin elasticity loss in columellar and alar areas

4. Surgical Approach

Upturned Nose Correction Technique and Process

a. Structural Reconstruction

  • Reused existing autologous rib cartilage

  • Added donated rib cartilage for stability

  • Strengthened nasal tip support with reinforced framework

b. Symmetry Improvement

  • Corrected alar asymmetry from prior nostril reduction

  • Balanced columellar height and tip projection

  • Filled depressed dorsum with cartilage and dermis

c. Functional Enhancement

  • Performed bilateral nostril correction

  • Composite skin grafting for thinned or scarred skin

  • Columellar skin grafting for better structure and aesthetics

This case required a delicate balance between stability and aesthetics due to the patient’s multiple surgical history. We combined autologous and donated materials to ensure both natural aesthetics and structural stability.
