Hello,

This is Dr. Cha-Young Kang, medical director of NOSELAB Clinic.

This patient came to us after multiple rhinoplasty surgeries over 25 years, presenting with an upturned nose and wide nasal bridge. In her last procedure, gluteal dermal grafts had been used but were largely absorbed, leaving her bridge flat, widened, and uneven.

Through Upturned Nose Correction, our goal was to lower the nasal tip, rebuild the bridge, and restore a balanced, natural contour using rib cartilage.

Surgical Background and Patient Goals for Upturned Nose Correction

Preoperative design photos for Upturned Nose Correction: frontal, left oblique, and left side views showing upturned tip and wide nasal bridge.
Preoperative design consultation photos: Frontal view, left oblique side view, and left side view.

Past procedures:

  • 25 years ago: Silicone implant (ineffective)

  • 19 years ago: L-shaped silicone (caused upturned tip)

  • 15 years ago: L-shaped silicone + ear cartilage

  • 10 years ago: Dermal graft (absorbed) + septal/ear cartilage

  • 2 years ago: Nasal filler injections

Preoperative views for Upturned Nose Correction: right side, right oblique, and nostril views showing upturned nasal tip and asymmetric nostrils.
Preoperative design consultation photos: Right side view, right oblique side view, and nostril view.

Patient’s goals:

  • Correct the upturned nasal tip

  • Narrow and smooth the bridge

  • Straighten the crooked upper vault

  • Restore the sunken middle vault

  • Improve overall facial balance

Detailed Surgical Plan for Upturned Nose Correction and Bridge Refinement

Foreign Material Removal

Removed residual fillers and absorbed dermal grafts that distorted the bridge.

Bridge Reconstruction

Used autologous rib cartilage to rebuild the middle vault and create a natural, straight dorsal line.

Septal Extension for Tip Correction

Lowered the upturned nasal tip, refined the nasolabial angle, and provided strong structural support for long-term stability.

Correction of Nostril Exposure

Lowered overly visible nostrils and restored left-right symmetry by repositioning the alar base and columella.

Rebuilding the Nasal Bridge with Rib Cartilage for Upturned Nose Correction

This case required restoring both height and proportion while ensuring structural integrity. The absorbed dermis had flattened and widened the dorsum, causing asymmetry. Using rib cartilage, we achieved:

  • Straighter, more defined dorsal contour

  • Balanced middle vault reconstruction

  • Natural height restoration without overcorrection

Straightening the Dorsum and Restoring Natural Facial Harmony

A key challenge in Upturned Nose Correction lies in rebuilding both internal and external support.

By realigning the deviated upper vault and correcting the nasolabial angle, we achieved facial balance and long-term tip stability.

Long-Term Stability and Structural Balance in Revision Rhinoplasty

Upturned Nose Correction requires more than pulling the tip downward — it demands structural rebuilding.

Using rib cartilage, we ensured stable, durable support that resists relapse or retraction.

At NOSELAB, every revision focuses on:

  • Removing unstable or absorbed materials

  • Rebuilding with autologous tissue

  • Prioritizing harmony over excessive height

  • Achieving long-term naturalness and safety
