Overview of This Asymmetrical Nostrils Correction Case
Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.
This patient presented with asymmetrical nostrils, skin necrosis, tip retraction, and chronic nasal obstruction after more than 20 years of repeated nasal procedures. We performed a closed rhinoplasty for asymmetrical nostrils correction using autologous rib cartilage.

Surgical History and Preoperative Analysis
History of Multiple Surgeries Before Asymmetrical Nostrils Correction
23 years ago: Silicone implant
20 years ago: Gore-Tex + ear cartilage + alar base reduction
13 years ago: Gore-Tex + ear cartilage
10 years ago: Nasal filler
7–8 years ago: Silicone + ear cartilage + septal surgery
3–4 months later: Revision for nasal redness (unknown details)
Preoperative Findings Related to Asymmetrical Nostrils
Frontal View: Uneven nostril exposure, deviated nasal bridge, thin dorsum
Side View: Retracted columella, upper lip protrusion, irregular dorsal line
Oblique View: Depression on left nasal bridge, suspected necrosis
Nostril View: Clear asymmetry and rightward deviation
Functionally: Severe left-sided nasal obstruction
Patient’s Expectations for Asymmetrical Nostrils Correction
The patient hoped to correct the asymmetrical nostrils, achieve a straighter nose, and restore a balanced, natural appearance. Functionally, the goal was to breathe clearly through both nostrils.
Surgical Plan – Closed Rhinoplasty for Asymmetrical Nostrils Correction
Septal Reconstruction and Tip Correction
Detected complete damage to septal cartilage
Autologous rib cartilage used to reconstruct and extend the septum
Upturned tip corrected via septal extension graft
Functional Correction Accompanying Asymmetrical Nostrils Correction
Septoplasty to straighten deviated septum
Radiofrequency turbinoplasty to reduce turbinate swelling
Nasal airflow restored
Aesthetic Refinement for Nostril Symmetry
Lowered columella for even nasal base
Filled depression with cartilage and dermal grafts
Dorsal line straightened
Asymmetrical nostrils corrected for balance and natural flow
Results After Closed Rhinoplasty for Asymmetrical Nostrils Correction
Frontal View: Significantly reduced nostril exposure and asymmetry

Side View: Smooth nasolabial angle and defined columella

Oblique View: 3D volume restored to left bridge

Function: Improved nasal breathing and septal support

Expert Review – A High-Difficulty Asymmetrical Nostrils Correction Case

This patient had undergone 6+ nasal procedures, causing cartilage damage, scarring, and severe nostril asymmetry. Using autologous rib cartilage, we reconstructed the septum and addressed the asymmetrical nostrils and upturned tip.
The nasal base was rebalanced by lowering the columella, restoring the nasolabial angle, and correcting soft tissue loss with dermal grafts.
🦴 At Nose Lab, all procedures are performed using the closed rhinoplasty technique.
