Bulbous and Wide Nose Revision Using Self-Rib Cartilage

Hello, this is Dr. Chayoung Kang, Director of Noselab Clinic, specializing in closed revision rhinoplasty in Korea.

Today, I will introduce a case of bulbous and wide nose revision combined with nostril asymmetry correction, performed using self-rib cartilage.


Preoperative Design and Assessment for Bulbous Nose Revision


The patient visited our clinic with a blunt and deviated nose that appeared asymmetrical.

The nasal skin was thick, and the previous silicone implant was bent to the right side.

In addition, excessive scar tissue in the right nose tip made the deviation more visible.

The columella had thickened, narrowing the nostrils and causing noticeable asymmetry.


Surgical Plan for Bulbous Nose and Nostril Asymmetry Correction


To correct these problems, I planned the following:

  • Remove scar tissue around the nasal tip to refine its blunt shape.

  • Perform a lateral osteotomy to correct nasal bone asymmetry and realign the deviated implant.

  • Reconstruct the lower columella using dermal graft and cartilage transplantation to improve scarring and restore proper shape.


Through these steps, both the shape and symmetry of the nostrils were addressed while improving nasal projection and tip definition.


Bulbous Nose Tip Reconstruction Using Self-Rib Cartilage


The key focus was the bulbous tip correction:

  • Removal of subcutaneous soft tissue,

  • Relocation of alar cartilage, and

  • Adding a three-dimensional effect to elevate the nasal tip naturally.


Since the patient had developed thick scar tissue and damaged alar cartilage, I reconstructed and relocated the nasal tip structure.

Finally, self-rib cartilage was used to support and enhance the nasal tip projection, resulting in a sharper, slimmer, and more balanced nasal contour.

General Overview and Postoperative Results


This revision surgery successfully corrected the bulbous nose, nostril asymmetry, and columella scarring.

Immediately after surgery, the nasal bridge appeared centered, and the tip projection improved naturally while maintaining harmony with the facial structure.
