Crooked Nose, Bulbous Tip, and Nostril Asymmetry Correction – Revision Rhinoplasty in Korea

Hello, this is Dr. Cha-Young Kang, head surgeon at Nose Lab Clinic.


Today, I’d like to present a crooked nose revision rhinoplasty case in Korea, involving a patient with a crooked nose, bulbous nasal tip, and nostril asymmetry. This was the patient’s third rhinoplasty after two previous surgeries.

The patient’s condition before surgery:

  • Crooked nasal bridge bending to the right

  • Nasal tip deviating to the left

  • Bulbous and undefined nasal tip

  • Nostril asymmetry with tilted columella

  • High dorsal starting point and droopy tip

  • Uneven, bumpy nasal bridge


Patient’s requests included: correction of crooked nose, smoothing the dorsal surface, maintaining nasal height, and resolving occasional unpleasant nasal odor.


Surgical Procedure in Crooked Nose Revision Rhinoplasty

The surgery was performed using a closed (endonasal) approach, with the following steps:


a) Septal Reconstruction with Autologous Rib Cartilage – Rib cartilage harvested through a small incision was used to reconstruct the septum and secure structural stability.


b) Septal Extension – Corrected the deviated nasal tip and provided central alignment.


c) Dermal Graft – Covered the protruding cartilage at the tip, creating a smoother skin surface.


d) Lateral Osteotomy – Straightened the crooked nasal bridge and refined the nasal framework.


e) Implant Replacement – Removed the previous silicone implant and replaced it with a new one for an even dorsal line.


f) Nostril Asymmetry Correction – Realigned the septum and repositioned the alar cartilage for improved symmetry.


All procedures were performed endonasally, leaving no external scars.

This crooked nose revision rhinoplasty successfully corrected structural and aesthetic issues caused by multiple previous surgeries. Septal reconstruction with autologous rib cartilage ensured stability, while osteotomy and implant replacement straightened and balanced the nose.


By refining the bulbous tip and correcting nostril asymmetry, we achieved both functional and aesthetic improvements. Using a closed approach allowed us to address these complex issues without leaving external scars.
