Long and Flat Nose Rhinoplasty Case – Closed Rhinoplasty Korea


Hello, this is Dr. Cha-Young Kang from NOSELAB Clinic.


Today, I’d like to introduce a closed rhinoplasty Korea case involving a patient who was concerned about a long and flat nose. The main request was to create a shorter-looking nose while reducing nostril exposure and correcting asymmetry.

  • Excessively exposed and asymmetrical nostrils

  • Nasal deviation to the left

  • High glabella starting point

  • Sagging columella with bumpy nasal bridge

  • Flat dorsum with a long philtrum and protruding mouth

  • Small nasolabial angle (<90°)

This patient had undergone two previous rhinoplasties:

  • 2 years prior: septoplasty + ear cartilage graft + silicone implant

  • 6 months earlier: implant replacement


Surgical Plan for Long and Flat Nose Correction


Septal Extension with Autologous Rib Cartilage

  • Strengthened nasal structure

  • Corrected nostril asymmetry

  • Stabilized columella alignment


Implant Replacement for a Natural Nasal Bridge

  • Removed the old silicone implant

  • Inserted a new implant only where necessary for natural definition


Nasolabial Angle and Nostril Symmetry Correction

  • Adjusted alar cartilages and performed cartilage tying

  • Refined columella position

  • Improved balance between nose and lips


Lowering the Glabella Starting Point

  • Designed a shorter nose impression

  • Enhanced overall proportion without unnecessary lengthening

Medical Insights on Long and Flat Nose Correction


This case emphasizes the balance between patient requests and surgical principles.While the ideal approach for visible nostrils often involves lengthening the tip, this patient strongly requested not to lengthen her nose. Therefore, we carefully followed her wishes while improving symmetry and proportion through alternative methods.


By lowering the glabella starting point, we created a visually shorter nose while enhancing facial harmony. Rib cartilage septal extension provided stability and reliable correction for nostril asymmetry.


This case highlights that rhinoplasty is not simply about changing nasal length or height but requires tailoring to individual needs and facial balance.

Conclusion


Cosmetic surgery often presents a dilemma between medical standards and patient desires. In this case, the patient’s sensitivity about nasal length guided the surgical plan. Through clear communication and a careful approach, we were able to achieve both subjective satisfaction and objective improvement.
