Why Closed Rhinoplasty Korea Was Essential in This Graft Absorption Case

Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.

In this post, I’m sharing a complex closed rhinoplasty Korea case. The patient had previously undergone multiple surgeries involving donor rib cartilage and silicone implants, which eventually led to graft absorption, nasal tip collapse, and visible scarring. To restore nasal structure and aesthetics, we performed a full reconstruction using autologous rib cartilage.

Surgical History and Key Problems

Closed rhinoplasty Korea – preoperative frontal, oblique, and side views showing tip shrinkage, nasal scar, and nostril asymmetry
Preoperative design consultation photos: Frontal view, left oblique side view, and left side view.

✅ Initial Surgery: Grafted rib cartilage (inflammation developed)

✅ 2018 Surgery: Silicone implant + auricular cartilage + donor rib cartilage

  • ⚠️ Main Issues:

    • Graft absorption and tip shrinkage

    • Shortened nasal length

    • Drooping tip and over-rotated appearance

    • Pinched and asymmetric nostrils

    • Dorsal scarring and skin thinning

Closed rhinoplasty Korea – preoperative right side, oblique, and nostril view showing nasal tip deformity, overexposed nostrils, and asymmetry
Preoperative design consultation photos: Right side view, right oblique side view, and nostril view.

Patient’s Goals with Closed Rhinoplasty Korea

  • Correct deformities caused by graft resorption

  • Restore natural nasal length and projection

  • Improve nostril symmetry

  • Smooth out scarring and skin irregularities

  • Achieve a stable, natural-looking result

Surgical Plan and Reconstruction Techniques

We used closed rhinoplasty Korea techniques with the following key steps:

  • Rib cartilage septal extension graft

  • Bilateral alar cartilage reconstruction

  • Correction of over-rotation and columellar angle

  • Scar smoothing with dermal + cartilage grafts

  • Tip projection and dorsal contour correction

Surgical Outcome of Closed Rhinoplasty Korea

Before & after closed rhinoplasty Korea using rib cartilage.
Frontal comparison before and immediately after closed rhinoplasty Korea using rib cartilage — showing tip refinement and dorsal correction.
Before Surgery (Left) / Immediately After Surgery (Right)

✅ Frontal view: Restored alar symmetry, softer nostril lines

Side profile comparison before and immediately after closed rhinoplasty Korea using rib cartilage — showing corrected nasal projection and tip rotation.
Before Surgery (Left) / Immediately After Surgery (Right)

✅ Side view: Improved nasal length and tip elevation

Oblique side view before and immediately after closed rhinoplasty Korea using rib cartilage — showing improved nasal tip projection and natural contour.
Before Surgery (Left) / Immediately After Surgery (Right)

✅ Nostril view: Balanced nostril shape and reduced overexposure

Nostril view before and immediately after closed rhinoplasty Korea — showing improved nostril symmetry and reduced flare using rib cartilage reconstruction.
Before Surgery (Left) / Immediately After Surgery (Right)

✅ Overall: Natural, refined profile with stable long-term support

Surgeon’s Commentaryy

Immediately after closed rhinoplasty Korea — frontal, oblique, and side view showing improved nasal tip projection, dorsal smoothness, and nostril balance.
Immediately after surgery: Frontal view, side view, and oblique side view photos.
Immediately after closed rhinoplasty Korea — nostril view showing improved symmetry and internal structural reconstruction.
Immediately after surgery: Nostril view photos.

This patient experienced chronic issues due to previous surgeries, especially graft absorption and nasal scarring. Through closed rhinoplasty Korea, we reconstructed the nasal framework using autologous rib cartilage—offering both aesthetic refinement and structural stability. This case highlights the reliability of using the patient’s own tissue in revision surgery, avoiding further foreign material complications.
