Today I will introduce a real case of columella correction after nasal contracture, a challenging revision rhinoplasty case.

Severe Nose Tip Cartilage Damage Due to Nasal Contracture

At Noselab, we frequently handle revision rhinoplasty surgeries, particularly for patients with nasal contracture deformities. One such patient had severe deformities in the nostrils and columella due to significant nose tip cartilage damage.

Understanding the Severity of Nasal Contracture

Nasal contracture often leads to more severe cases when septal cartilage is damaged. In this particular case, the nose tip cartilage was almost unrecognizable due to the near absence of septal cartilage. This condition likely resulted from inflammation, which caused the nasal contracture deformity and subsequent retraction of the columella, narrowing the nostrils.

Surgical Findings and Reconstruction

The surgical findings were more challenging than anticipated. With almost no septal cartilage remaining, a complete septal cartilage reconstruction was necessary. We used autologous septal cartilage to rebuild the structure. Even after reconstruction, the soft tissue deformity in the columella was severe, requiring additional cartilage to lower the columella and strengthen the nose tip’s support structure.

The columella, initially invisible from the side profile, was significantly improved post-surgery. The columella was lowered, creating a natural nasolabial angle. The previously droopy nose tip was also enhanced.

Frontal View Transformation

From the front view, the blunt nose tip and retracted columella were corrected, giving the nose a more natural appearance. The columella, which had pushed inward and widened the nostrils, was effectively restored post-surgery.

Dr. Kang’s Opinion on Columella Deformation Due to Nasal Contracture

Inflammation and contracture are major concerns for rhinoplasty patients and surgeons. Inflammation can deform the columella, a small but crucial part of the nose. Most hospitals perform open surgery, which can result in scars and poor blood circulation in the columella. If contracture deformation occurs, it becomes highly noticeable and difficult to treat.

At NOSELAB, we utilize a technique called inner open surgery (Noselab’s unique term for closed rhinoplasty). This method allows safer access to the affected areas, resulting in better outcomes. Preventing inflammation is crucial, but when it occurs, our mission is to minimize deformation through appropriate treatment and fully restore the affected parts via inner open surgery.
