• 10 years ago: Nasal filler and PDO thread lift (Misko)

• 9–10 years ago: Silicone implant with AlloDerm graft

• 8–9 years ago: Revision rhinoplasty using silicone for crooked nose correction

Patient’s Requests:

  • Raise the nasal height naturally

Surgical Plan and Structural Approach to Droopy Columella Correction


This was a high-difficulty revision surgery that required multiple corrections at once — implant removal, droopy columella correction, crooked nose realignment, and nostril symmetry restoration.


Removal of Previous Implants and Tissue Reconstruction

All previous materials — silicone, AlloDerm, filler, and PDO threads — were completely removed.The internal nasal framework was rebuilt with autologous rib cartilage, providing stable, long-term support.


Droopy Columella Correction and Nasolabial Angle Refinement

The droopy columella was elevated to a natural position, and the nasolabial angle was adjusted for a smoother, refined side profile.


Crooked Nose Realignment with Septal Reconstruction

A lateral osteotomy was performed to correct the deviation.

The septum was reconstructed with rib cartilage to restore midline alignment and improve internal support.


Nostril Lowering and Symmetry Correction

The alar cartilage was precisely repositioned, and rib cartilage grafts were added to lower nostril margins, reducing nostril exposure and restoring symmetry.
