Unexpected Foreign Material Removal with Rib Cartilage Reconstruction – Revision Case Study

Hello, this is Dr. Cha-Young Kang, Chief Surgeon at Nose Lab Clinic.


Today, I am sharing a unique revision rhinoplasty case that involved unexpected foreign material removal and rib cartilage reconstruction. This patient, who believed they had only undergone nostril reduction, was found to have hidden Medpor and other unknown substances during surgery.

1. Surgical History and Patient Background

  • Multiple filler injections for nasal contouring

  • Nostril reduction surgery (patient unaware of any implants or additional procedures)


2. Unexpected Findings During Foreign Material Removal

  • Severe scar tissue and adhesion

  • Undisclosed Medpor implant for septal extension

  • Unabsorbed synthetic material in the glabella (between the eyebrows)

  • Nylon sutures securing nasal tip cartilage

3. Surgical Plan and Foreign Material Extraction Approach

a. Foreign Material Removal

  • Complete removal of Medpor

  • Extraction of glabellar foreign material

  • Removal of nylon sutures

  • Deep cleansing of residual substances


b. Rib Cartilage Reconstruction

  • Autologous rib cartilage graft for septal support

  • Lateral osteotomy for nasal bone alignment

  • Dorsal and tip refinement for natural shape


c. Structural Refinement

  • Strengthening nasal framework

  • Creating a smooth nasal bridge transition

  • Improving nasal proportions

This was a highly complex case involving the removal of unexpected foreign material, including Medpor and synthetic debris. These had caused adhesions and unnatural nasal structure. Through careful planning and rib cartilage reconstruction, we successfully restored nasal structure and aesthetics.
