Long and Wide Nose Correction – Closed Rhinoplasty Korea

Hello, this is Dr. Chayoung Kang, Director of Noselab Rhinoplasty Clinic. Today, I will introduce a long nose correction case of a patient who was concerned about a long and wide nose, mainly due to her droopy nasal tip and nostril asymmetry.


This case demonstrates how closed rhinoplasty can effectively address complex concerns such as a wide nasal base, droopy tip, and nostril asymmetry, while achieving natural harmony.


Patient’s Concerns and Preoperative Analysis – Long Nose Correction Case


The patient’s main requests were:

  • Improving droopy nose

  • Natural shape

  • Increased nose height

  • Straight nose shape

  • A nose bridge that is not too thin

  • Round nose tip

Surgical plan


​We established the following surgical plan taking into account the patient’s condition and needs:


a) Nose tip correction:


  • Donated costal cartilage and septal cartilage were used as per the patient’s request.

  • Septal extension surgery


​b) Nose bridge correction:


  • Lateral osteotomy to improve wide appearance.


​c) Improvement of nostril asymmetry:


  • Nasal cartilage repositioning

  • Application of cartilage tying technique


Originally, we recommended using autologous costal cartilage to straighten the tip of the nose, but at the patient’s request, we decided to use donated costal cartilage.


​Surgical details


The following surgeries were performed using a closed rhinoplasty approach:


a) Septum and nose tip correction:


  • During the surgery, we found that the nasal septum cartilage was deviated to the right, but its size was sufficient.

  • The nose tip surgery was performed using donated costal cartilage and septal cartilage together.

  • The blunt and wide appearance of the nose tip improved while maintaining its roundness as much as possible.


b) Nose bridge correction:


  • Through the lateral osteotomy, the wide appearance improved. c) Improvement of nostril asymmetry:

  • Asymmetry was improved by repositioning the alar cartilage and applying the cartilage tying technique.


​During the surgery, the shape of the tip of the nose was given special attention. The degree to which the tip of the nose is round or pointed is very subjective, so it is difficult to get it exactly right. In particular, if the nose is not large, and the tip of the nose is raised, the skin becomes thinner, which can make it look pointy. To prevent this, we carefully adjusted the width of the cartilage at the tip of the nose and meticulously shaped the columella to prevent it the tip to look pointy as much as possible.

  • The wide nose became slimmer

  • A more balanced face as the lenght of the nose was improved.

  • Improvement of the nostril asymmetry.

  • As the droopy nose was adjusted and raised, the facial harmony became enhanced.

  • Reduction of the protruding mouth

  • Natural adjustment of the nose bridge starting point

  • Enhancement of the three-dimensionality.

  • The long appearance improved

  • As the nose tip and bridge became more balanced the overall apperance chnaged to be more harmonious.

Overview


This case is one where both a long and wide nose improved at the same time. We paid great attention to achieving a natural result while reflecting the patient’s needs as much as possible. The use of donated costal cartilage and septal cartilage together was a choice to achieve stable results while respecting the patient’s request. Through septal extension surgery, we tried to improve the droopiness of the nasal tip while maintaining a its round shape. Improving the wide nose appearance through lateral osteotomy played a significant role in refining the overall contour of the nose. In addition, the alar cartilage repositioning and tying were used to correct the nostril asymmetry problem.


In particular, what was important in this case was to maintain the ‘naturalness’ that the patient wanted. We carefully adjusted the tip of the nose to make it look high without making it look sharp, and we also paid attention to making sure that the bridge of the nose did not look too thin.


By performing this surgery through the closed rhinoplasty method, which does not leave external scars, we were able to increase the patient’s satisfaction, while ensuring a fast recovery period.
