Nose Hump Correction with Functional Improvement through Closed Rhinoplasty in Korea

Hello, this is Dr. Chayoung Kang, Director of Noselab Rhinoplasty Clinic. Today we will introduce the case of a non-implant rhinoplasty performed to correct a long, hooked nose with a nasal hump, while also addressing breathing difficulties caused by nasal congestion.


This patient visited our clinic to improve her functional breathing issues as well as to transform her arrow-shaped nose into a softer and more feminine appearance.

CT scan findings:


Left sided septal deviation

Enlarged inferior turbinates on both sides

Surgical Details


The following surgeries were performed using the closed rhinoplasty approach:


a) Correction of hump and deviated nose:


  • Shaving of the nasal bone to improve the hooked nose shape.

  • Performance of a lateral osteotomy to improve the nasal hump.


b) Septoplasty:


  • Corrected the septum deviated to the left.

  • The septal cartilage was very weak and bent, so we performed a partial reconstruction surgery to move it to the right side enhancing stability.


c) Submucosal resection of inferior turbinates:


Reduction of the volume of the hypertrophied inferior turbinate using a highfrequency laser to improve the nasal congestion symptoms.


d) Septal extension garft surgery:


  • 1mm raise of the nose tip towards the bridge of the nose to improve the lenght of the nose.


e) Nasal cartilage repositioning and tying


  • Improvement of the nostril asymmetry through the nasal alar cartilage repositioning and cartilage tying.


f) Raising the nasal bridge without implants


  • As per request of the patient, we used autologous cartilage instead of silicone implants to soften the parts that were off and made them look more natural.


​All procedures were performed using a closed approach, completing the surgery without external scars.

  • The appearance become more harmonious as we straightened the deviated and long nose.

  • Chnaged the arrow-shaped nose into a more natural nose shape.

  • Improvement of the nostril asymmetry.

  • Improvement of the nose appearance by increasing the nostril exposure appropriately.

  • Balance improvement by raising the nose tip, improving the droopiness.

  • By correcting and fixing the columella in place, the side profile became more sophisticated.

  • The lenght of the nose improved as the nasolabial angle.

  • The nose line became more natural looking, after correcting the long and hooked nose.

  • The harmony between the nose tip and bridge improved in general as the appearance became more balanced.

Overview


In this case, complex problems of a long, humped and deviated nose presenting nasal congestion were solved at the same time. In particular, at the patient’s request, we performed the surgery without implants, and were able to achieve natural results. Through septoplasty and inferior turbinate reduction, we improved the nasal congestion problem and at the same time, made a big change in the cosmetic aspect. By correcting the hump and the deviation, we made the overall nose line softer and more feminine, and through septal extension, we were able to adjust the lenght of the nose.


It is particularly noteworthy that the effect of a higher and more natural nose was possible to create despite the fact that it was performed without implants. Raising the bridge of the nose using cartilage is a good way to obtain satisfactory results while reducing the risk of side effects that may occur when using prosthetics.


Correcting the asymmetry of the nostrils through alar cartilage repositioning and cartilage tying also played a major role in


improving the overall balance of the nose. This created a more refined and balanced appearance when viewed from the front.


Through the closed rhinoplasty approach, we were able to ensure a shorter recovery period, and avoid leaving external scars, which increased the patient’s satisfaction.


All procedures were performed using a closed approach, completing the surgery without external scars.
