Introduction to the Case


Hello, this is Dr. Cha-Young Kang, Chief Director at Nose Lab Clinic.


In this blog, I will share a complex case of revision rhinoplasty for a contracted and deviated nose following four previous surgeries. The patient suffered from a retracted (upturned) tip, nasal asymmetry, and filler-related distortion.


📍 Surgical History

  • 6–7 years ago: Silicone implant + ear cartilage

  • 5 years ago: Implant replacement + septoplasty

  • 4 years ago: Implant removal + autologous dermis + rib cartilage + alar base reduction

  • 2 years ago: Tip revision with rib cartilage

  • 1 year ago: Filler injection for low nasal bridge


🔍 Initial Observations (Revision Rhinoplasty for Contracted and Deviated Nose)

Frontal View:

  • Crooked bridge, bulky dorsum, bulbous tip

  • Alar base asymmetry, smaller left nostril


Side View:

  • High radix, drooping tip, small nasolabial angle


Nasal Base:

  • Asymmetrical nostrils, left side contracture


✨ Patient's Goals

  • Correct deviated bridge

  • Lower upturned tip (top priority)

  • Improve symmetry and projection

  • Use silicone implant for structure


💉 Surgical Plan – Revision Rhinoplasty for Contracted and Deviated Nose

1. Structural Correction:

  • Septal extension using autologous rib cartilage

  • Alar repositioning and nostril reshaping


2. Aesthetic Refinement:

  • Realignment of bridge

  • Nasolabial angle correction

  • Natural projection of nasal tip


3. Implant & Tissue Management:

  • Removal of filler and foreign material

  • Silicone dorsal implant for ideal height

  • Tension-free reconstruction to prevent recurrence

  • Straight, balanced nasal line

  • Improved alar symmetry

  • Even nostrils

  • Corrected pinching and contracture

This revision rhinoplasty for contracted and deviated nose involved high difficulty due to prior surgeries and nasal tissue damage. The focus was to stabilize the lowered tip and improve symmetry using autologous rib cartilage and silicone. We also carefully removed residual filler to create a clean foundation for reconstruction.
