Surgical History and Complications from Revision Rhinoplasty Silicone Extrusion
The patient had an L-shaped silicone implant inserted in Korea 30 years ago, followed by a suspected revision rhinoplasty using silicone in Japan 20 years ago. Over time, the patient developed visible silicone extrusion and chronic rhinitis symptoms, such as severe nasal discharge and frequent sneezing.

Patient Goals for Revision Rhinoplasty Silicone Extrusion Correction
The patient’s primary goals were to remove the silicone extrusion, treat chronic rhinitis, and achieve a natural, balanced nose line. She was open to a rounded nasal tip if it meant a softer and more natural result.
Surgical Approach
Removing Silicone and Reconstruction in Revision Rhinoplasty Silicone Extrusion
We completely removed the aged silicone implant and meticulously reconstructed the damaged cartilage using autologous tissue. At the same time, we performed surgical treatment for rhinitis to address functional symptoms.
Functional Improvement
We corrected the nasolabial angle, reconstructed the deviated septum, and restored normal breathing.
Aesthetic Refinement
We created a natural nasal tip, adjusted the overall nasal structure for balance, and established a stable nasal framework.
Surgical Results – Revision Rhinoplasty Silicone Extrusion
Frontal View
Silicone extrusion was resolved, resulting in a natural, balanced nasal tip and harmonized contour.

Side View
The nasolabial angle improved, the tip line became softer, and the profile harmonized with facial proportions.

Oblique View
The 3D contour and projection improved, structural support stabilized, and overall balance enhanced.

Nostril View
Nostril asymmetry was corrected, the columella straightened, and scar tissue removed.

Functional Improvement
Rhinitis symptoms significantly decreased, breathing improved, and nasal discharge and sneezing reduced.
Expert Commentary on Revision Rhinoplasty Silicone Extrusion

Long-term silicone implants often cause extrusion, tissue thinning, and infection. In revision rhinoplasty silicone extrusion cases, the challenge is to restore both aesthetics and function.
Key steps included safe removal of the aged implant, precise reconstruction with autologous tissue, and functional improvements through septoplasty and rhinitis treatment. The result was a natural, balanced nose and a significant improvement in quality of life. NoseLab performs all procedures with a closed approach.
