3. Surgical Plan
The treatment approach required a combination of functional and aesthetic procedures for a well-rounded outcome:
Septoplasty: Correcting septal deviation to improve breathing.
Turbinate Reduction with Radiofrequency Laser: Treating turbinate hypertrophy to relieve rhinitis symptoms.
Septal Extension Graft: Using the patient’s septal cartilage to stabilize and refine nasal shape.
Dorsal Hump Reduction: Smoothing the nasal bridge to create a balanced contour.
Lateral Osteotomy: Narrowing the bridge to harmonize with the nose’s new structure.
Silicone Implant for Bridge Height: Achieving a subtle enhancement to the nasal bridge for balance.
Key Considerations for Long Nose Correction
Achieving balance in a long nose requires more than just reducing its length; it involves refining its shape to harmonize with the facial proportions. Here’s what we focused on in this case:
Facial Proportion Consideration: Ensuring that nasal length and shape complemented the patient’s overall facial structure.
Bridge Starting Point: Setting the bridge height at a level that blends naturally with the brow.
Nostril Aesthetics: Designing the nostrils to minimize prominence from the front view.
Nasolabial Angle Adjustment: Creating a subtle upward tilt at the base of the nose to visually reduce perceived length.
Columella Adjustment: The final touch involved refining the columella to ensure the new nose length and shape suited her face.

4. Post-Surgical Results
The results achieved were in line with the patient’s goals for a natural and balanced look:
Front View: Reduced broadness and better symmetry in the nostrils, with a straightened nasal line.
Side View: A smooth nasal bridge, with the hump reduced and the nasal tip subtly lifted.
Nostril View: Improved symmetry, with a balanced nasal base and reshaped lateral nasal walls.
