Scar Tissue Nasal Reconstruction with Rib Cartilage: Restoring Form and Function


Hello, I’m Dr. Cha-Young Kang, Director of NoseLab Clinic.


Today, I’d like to present a complex case of scar tissue nasal reconstruction. The patient sustained a nasal fracture during military service while playing football, and subsequent revision surgeries left him with persistent scar tissue, nostril asymmetry, and impaired function. Using autologous rib cartilage, we achieved full structural and aesthetic restoration.

  • Military trauma: Nasal bone fracture during football

  • 6 years ago: Tip and hump correction with ear cartilage

  • 2 years ago: Nasal bone reconstruction

  • 8 months ago: Silicone dorsal implant with functional surgery (excluding tip)

Despite these attempts, the patient developed internal fibrosis, nostril asymmetry, and an unstable nasal framework.

Rib Cartilage Nasal Reconstruction Strategy


  1. Framework Reconstruction: Removed silicone implant; harvested rib cartilage to rebuild structure; reinforced weakened bone and cartilage.

  2. Septal Realignment: Corrected deviation, improved airflow, balanced nostril symmetry.

  3. Nasolabial Refinement: Adjusted angle for a softer, more proportional facial silhouette.

  4. Scar Tissue Management: Removed fibrotic tissue, restored blood supply and tissue flexibility, minimized risk of contracture.
