Surgical History of Silitex Implant Removal Rhinoplasty Revision

The patient underwent primary rhinoplasty in 2019 using a Silitex implant, a combination of Gore-Tex encasing silicone. Over time, functional and aesthetic issues developed, leading to the need for a Silitex implant removal rhinoplasty revision.

Initial Assessment Before Silitex Implant Removal

Frontal View Findings

  • Excessive nostril exposure, implant translucency at the nasal dorsum, high nasal bridge starting point, and long philtrum created an artificial appearance.

Side View Findings

  • Overly high nasal root at the glabella, short upturned nasal tip, depressed columella, and narrow nasolabial angle gave the upper lip a protruding look.

Oblique View Findings

  • Prominent upper lip protrusion, unnatural glabella-to-dorsum transition, and columellar retraction were noted.

Nostril View Findings

  • Significant nostril asymmetry and scar tissue on the right columella were visible.

Patient Goals for Silitex Implant Removal Rhinoplasty Revision

The patient aimed to correct the nasolabial angle, reduce lip protrusion, lower the nasal bridge starting point, achieve a smooth transition at the nasal root, refine the nasal tip, reduce nostril visibility, and improve columella support.

Surgical Approach in Silitex Implant Removal Rhinoplasty Revision

Implant Removal

  • Safe, complete removal of the Silitex implant with attached Gore-Tex layers, along with excision of scar tissue and damaged soft tissue.

Structural Reconstruction

  • Autologous rib cartilage for tip projection and support, custom silicone implant for smooth dorsal line, and rib cartilage grafts for columella and nasolabial angle correction.

Overall Refinements

  • Facial fat grafting for contour harmony, lowering nasal root starting point, and balancing nasal proportions with facial features.

Silitex implants, combining Gore-Tex and silicone, adhere strongly to tissue, making removal technically demanding. Incomplete removal can cause complications. This case succeeded by safely extracting the implant and reconstructing with rib cartilage and silicone, restoring both function and aesthetics.
