Today, I’d like to introduce a complex case of Upturned Nose Correction using donor rib cartilage in a patient who had previously undergone multiple revision rhinoplasties. The patient presented with a wide nasal bridge and excessive tip rotation due to prior surgeries and thread lifting.

1. Surgical History – Multiple Revisions Before Upturned Nose Correction


✔ 10 years ago: Auricular cartilage + silicone implant

✔ 2 months later: Implant removal due to foreign body reaction

✔ 5 years ago: Silicone + septal cartilage graft

✔ 3 years ago: Silicone + donor rib cartilage graft

✔ Thread lifting: led to foreign body reaction

✔ 6 months ago: Complete removal + autologous rib cartilage reconstruction

Step 1 – Pre-Surgical CT Analysis & Material Removal

  • CT scan to detect residual foreign bodies

  • Removal of threads, filler, and remaining synthetic materials

Step 2 – Wide Nasal Bridge Refinement

  • Osteotomy to narrow the bridge

  • Sculpting of nasal dorsum

  • Donor rib cartilage for refined, smooth contour


Step 3 – Upturned Nose Correction

  • Adjust tip rotation to a natural position

  • Preserve autologous rib cartilage

  • Additional donor rib cartilage grafts for tip and support

  • Septal reconstruction for long-term stability

This was a high-difficulty case due to multiple surgeries, severe implant-related complications, and significant asymmetry.


The success of the Upturned Nose Correction was achieved through:
