Upturned Nose Correction: The Challenge of a Sixth Rhinoplasty

Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic. Today, I'd like to share the complex journey of a patient who underwent her sixth rhinoplasty, focusing on Upturned Nose Correction to achieve a natural, straight nasal line with a subtle curve.


Patient Background – Multiple Surgeries Before Upturned Nose Correction

This patient had undergone five rhinoplasty procedures between 2009 and 2014, attempting methods such as septal cartilage, donor rib cartilage, ear cartilage, and silicone implants. Unfortunately, each procedure left her dissatisfied. Eight years ago, she also received a filler injection, which provided only temporary improvement.

During our consultation, we identified the following concerns:

  • Upturned nasal tip

  • High starting point of the nasal bridge

  • Lack of a soft, natural curve in the nasal line

  • Nostril asymmetry

  • Visible alar scars from aggressive nostril and philtrum reductions

  • Columella scars from multiple open rhinoplasty procedures

Surgical Plan for Effective Upturned Nose Correction

We carefully developed a surgical plan tailored to her needs:

  1. Complete removal of all existing foreign materials (filler, silicone implants)

  2. Reuse of previously harvested tissues (rib, septal, ear cartilage)

  3. Harvesting additional rib cartilage as the primary graft material

  4. Septal extension graft to refine and stabilize the nasal tip

  5. Placement of a new silicone implant, positioned lower than previous implants


This procedure was particularly challenging due to weakened tissues and extensive scar formation. However, by proceeding step by step, we were able to achieve safe and precise Upturned Nose Correction.


 Post-Surgical Results of Upturned Nose Correction

Immediately after surgery, several improvements were visible:

  • The upturned nasal tip was corrected to a more natural position

  • The nasal bridge starting point was lowered to pupil level

  • The nasolabial angle was adjusted to ~95 degrees, creating a natural feminine profile

  • The crooked portions of the nose were straightened

  • Nostril symmetry was restored


Her nose now displayed the subtle, straight nasal line she desired, with a refined tip and smooth bridge.
