Hello, this is Dr. Cha-Young Kang, head surgeon at Nose Lab Clinic.


Today, I would like to present a rhinoplasty case involving a patient with multiple concerns: an upturned nose, small nose, nasal obstruction, and protruded lips. This case demonstrates how both function and aesthetics can be improved through a closed (endonasal) rhinoplasty approach.

  • Frontal view: Excessive nostril exposure, short and wide nose, low glabella.


  • Oblique view: Protruded lips, upturned nose, flat glabella.

Surgical Plan for Upturned Nose Correction and Nasolabial Angle Adjustment

  • Septoplasty for deviated septum correction

Closed Rhinoplasty Procedure Using Rib Cartilage

The surgery was performed with a closed approach. Septoplasty corrected the deviated septum, improving airflow. Turbinate reduction alleviated rhinitis symptoms. Rib cartilage reconstruction strengthened the septum and corrected the upturned tip. The columella was lowered to refine the nasolabial angle. Osteotomy narrowed the wide bridge, and a silicone implant created a straight dorsal line.

This surgery achieved both functional and cosmetic goals. Rib cartilage was key for stable support, while columella and nasolabial angle correction enhanced facial harmony. All performed with a closed approach, ensuring no external scars and quicker recovery.
