Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.
In this case, we corrected a bulbous nose and dragon nose contour combined with columella retraction through a closed rhinoplasty technique, aiming for natural and balanced results.

Surgical History:
3 years ago: Nasal filler injections (1-year and 5-year fillers, multiple sessions)
8 months ago: Philtrum reduction surgery
Goals:
Correct bulbous nasal tip and dragon-like nose shape
Address columella retraction

Pre-Surgical Analysis
Frontal View: Bulbous and wide nasal tip, dragon-like contour, low nasion, mild deviation of bridge
Side View: Low nasion, retracted columella, protrusive lip profile
Nostril View: Nostril asymmetry
Surgical Plan for Bulbous Nose Correction
To achieve the desired transformation, we created a precise surgical plan.
Bulbous Nose Correction:
Septal extension graft for support and definition
Removal of excess subcutaneous tissue (with caution to prevent complications)
Repositioning and suturing of alar cartilage to refine the tip
Dragon Nose Refinement:
Repositioning alar cartilage to soften the dragon-like contour
Adding septal cartilage to alar structure to lower nostrils and create harmony
Tip Elevation and Bridge Correction:
Controlled septal extension graft using septal + donor rib cartilage for tip elevation
Removal of residual filler
Insertion of silicone implant to achieve a straight and balanced bridge line

Nasal Contour: Refined bulbous and deviated nose, resulting in natural balance
Columella: Lowered columella improved the nasolabial angle and overall profile
Bridge Proportion: Raised nasion improved facial proportions
