Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.

Today, I’d like to share a nostril stenosis reconstruction case that resulted from multiple previous surgeries, including nostril reduction and philtrum shortening. This case highlights the importance of cautious surgical planning and the delicate approach required to correct nostril constriction while preserving natural aesthetics and functional breathing.

1. Initial Assessment – Functional & Aesthetic Concerns in Nostril Stenosis Reconstruction

Frontal, left side, and oblique side view photos of a patient before nostril stenosis reconstruction, used for surgical design consultation.
Design Consultation: Frontal View, Left Side View, and Oblique Side View Photos

Frontal View

🔹 Bulbous, wide nasal tip

🔹 Asymmetrical nostrils

🔹 Pinched nasal tip

🔹 Mildly deviated nasal bridge

Right side, oblique side, and nostril view photos of a patient before nostril stenosis reconstruction, taken during surgical design consultation.
Design Consultation: Right Side View, Oblique Side View, and Nostril View Photos

Nostril View

🔹 Significant nostril asymmetry

🔹 Collapsed left nostril due to prior nostril reduction

🔹 Leftward deviation of the columella

🔹 Internal scar tissue obstruction in the left nostril

2. Surgical Plan for Nostril Stenosis Reconstruction

✅ Reconstruct the septum and columella using autologous rib cartilage

✅ Correct the deviated bridge via lateral osteotomy

✅ Expand the collapsed nostril using local flap (rotation flap technique)

✅ Rebuild the nasal base to restore structural integrity

✅ Remove excessive internal scar tissue

✅ Adjust nasolabial angle for improved facial harmony

3. Key Surgical Procedures in Nostril Stenosis Reconstruction

Structural Reconstruction

🔹 Severe septal cartilage damage and weakened alar cartilage were rebuilt using autologous rib cartilage for long-term stability.

Tip & Nostril Refinement

🔹 Extensive scar tissue was excised to eliminate contracture and nasal obstruction.

🔹 Rotation flap technique expanded the left nostril and improved symmetry.

Functional Nasal Correction

🔹 Left nasal valve collapse was repaired, significantly improving airflow.

🔹 Nasolabial angle adjustment refined profile aesthetics.

4. Post-Surgical Outcomes

Frontal view comparison of a patient before surgery (left) and immediately after nostril stenosis reconstruction (right).
Before (Left) / Immediately After (Right)

Frontal View

✔ Balanced nostril symmetry

✔ Natural nasal tip contour

✔ Straighter columella alignment

Side profile comparison before (left) and immediately after nostril stenosis reconstruction surgery (right), showing improved nasal tip projection and nostril opening.
Before (Left) / Immediately After (Right)

Side Profile

✔ Proportional nasal tip projection

✔ Softer, more feminine nasolabial angle

Oblique side view comparison before (left) and immediately after nostril stenosis reconstruction surgery (right), showing improved nostril opening, nasal base symmetry, and refined tip contour.
Before (Left) / Immediately After (Right)

Three-Quarter View

✔ Enhanced nasal structure without pinching

✔ Harmonized dorsal line

Nostril view comparison before (left) and immediately after nostril stenosis reconstruction surgery (right), showing corrected nostril asymmetry, widened nasal openings, and restored nasal base contour.
Before (Left) / Immediately After (Right)

Nostril Correction

✔ Improved left nostril opening

✔ Removal of internal scar tissue

✔ Stable, natural nasal base reconstruction

5. Expert Insights – Challenges in Nostril Stenosis Reconstruction

Preoperative nostril view showing severe nostril stenosis (left) and intraoperative scene (right) during nostril stenosis reconstruction surgery.

Why is Nostril Stenosis Difficult to Repair?

1️⃣ High risk of recurrence due to scar contracture

2️⃣ Possible airway compromise if overcorrected

3️⃣ Structural limitations from prior surgery

Keys to Success

✅ Minimal and precise scar tissue removal

✅ Adequate structural support with autologous cartilage

✅ Natural contour restoration without overcorrection

6. Final Thoughts & Patient Advice

Immediately after surgery: frontal view, side view, and oblique side view showing corrected nostril asymmetry and improved nasal tip shape.

Immediately after surgery: nostril view showing improved symmetry and reconstructed nasal base.

🔹 Avoid aggressive nostril reduction to prevent irreversible complications.

🔹 If experiencing nostril collapse or breathing issues, seek specialized revision surgery.

At Nose Lab Clinic, we specialize in closed rhinoplasty for complex nostril stenosis reconstruction, ensuring both functional and aesthetic results.

Nose Lab performs all surgeries using the closed rhinoplasty approach.
