Patient’s Requests:

Achieve a rounded, gently lowered columella
Avoid an excessively elongated columella
Correct asymmetry in nostrils
Maintain the existing silicone implant without altering the nasal bridge or height

2. Surgical Plan and Execution of Customized Rhinoplasty with Specific Requests

Procedures Performed:

Columella Adjustment with Donor Rib Cartilage: To achieve a rounded shape and reduce columella visibility.
Right Alar Cartilage Augmentation: Donor rib cartilage added to extend right alar cartilage for balanced nostril appearance.
Scar Tissue Removal: A triangular scar below the left columella was removed to improve smoothness and symmetry.
Correction of Nostril Asymmetry: Enhanced symmetry for a more natural, balanced look.

3. Surgical Outcome

Front View: Nostril asymmetry was significantly improved, achieving a harmonious appearance.
Columella Correction: Subtly lowered to meet the patient’s preference for a rounded shape.
Scar Reduction: Triangular scar under the left columella successfully removed, enhancing overall balance.
